Introduction:

Entaliv is a type of classification of antiviral drug and indicated to use for the treatment of HBV in adults and children (>2yrs). It is not a cure for hepatitis but inhibits the virus from multiplying in body. Whether entaliv is not known will prevent cirrhosis or liver cancer.

Mechanism of action:

Entaliv, a nucleotide analogue these agents used to fight against the infection of HBV, HCV, herpes simplex and HIV. When introduce phosphate group in a molecule, they act as antimetabolities similar to nucleotides to be incarcerated into DNA strands growth, but Entaliv work as chain terminators and blocks the DNA viral polymerase. Entaliv stops the all 3 steps of reproduction process:
Base stimulate
Appropriate RNA messenger produce negative strand of reverse transcription
Production of positive strand of HBV DNA
Hence no specific DNA viral and also prevent mitochondrial DNA.Entaliv decrease the quanity of HBV in the blood by lowering the ability to reproduce and attack new cells.

Indication:

Entaliv 0.5mg in adult and children < 2yrs, X vir mainly used for treatment of long lasting Hepatitis B infection. The infection caused by Hepatitis B virus in liver. This infection will damage the liver and cause liver failure or liver cancer. The reducing of hepatitis B virus quantity in body is carried out by Entaliv tablet. Entaliv, after liver transplantation of the replication of HBV is prevented. X vir is used for the selected for resistance to lamivudine and emtricitabine in HIV, hence its not prescribed for both HIV-HBV patient without fully stopping Entaliv drug.

Pharmacokinetic :

Absorption:
By oral administration, Entaliv drug has peak plasma concentration ranges between 0.5 &1.5 hrs. In fasted state absorption is delay with standard high meal or light meal. So entaliv 0.5mg taken with food.

Distribution:
Binding of Entaliv to plasma protein is approx 13%

Metabolism:
Entaliv metabolized at glucuronide and sulfate conjugates. There is no acetylated or oxidative metabolites observed.

Elimination:
Entaliv eliminated via kidney (62%) and liver (73%). The terminal excretion of Entaliv half life is 24hrs.

Dosage And Administration:

Entaliv should administrated on an empty stomach interval of (2hr before and after the meal) Every day tablet should take on same time for accurate result Discontinue of Enteliv tablet will affects the effectiveness of treatment

Missed dose:
If dose of Enteliv is missed then take as soon as possible before next dose, but if time of next dose then allow the missed dose to skip and come to daily base schedule dosing. Avoid taking 2 doses at a time.

Precaution:

Entaliv co administration other drugs( lamivudine, adefovir, dipivoxil, or tenofovir) will result no drug interaction and affect the renal function, hence patient should be monitored regularily. The patient who discontinue the anti hepatitis treatment ( Entaliv) should check the hepatic function follow up closely then restart of anti viral drug may be warned.

Overdosage:

Cronvir overdose, contact your local poison control center or emergency department which indicate bad reaction to the drug. Cronvir include asthma; chest discomfort; fever; rashes in skin; blue or gray skin color; seizures; or swelling of face, lips, tongue, or throat. Trouble breathing. fast heartbeat,severe belly pain. stomach upset or throwing up,diarrhea,dyspepsia. Dark urine or yellow skin or eyes. Fatigue etc.
